Catholic Charities of Baltimore, Community Housing - Hosana House is seeking a Program Coordinator to coordinate client services and facility operations for either the Holden Hall Community Service program, which serves single men experiencing homelessness, or the Hosanna House Community Housing program, which serves single women experiencing homelessness. This position provides case management services that support the development and achievement of individualized client service plans, collaborates with community partners, volunteers, and City agencies, and prepares accurate and timely program reports.

Community Housing is a combination of multiple Permanent Supportive Housing (PSH) programs which includes, Project REACH, Project Fresh Start, Project Promise, Holden Hall and Hosanna House, which compassionately houses over 100 individuals living in communities throughout Baltimore City and Baltimore County. It offers residents who have experienced unstable housing with a disability affordable long-term housing with supports. Their lives are improved by offering case management and supportive services with the goal that every person can realize his/her full potential.

The work schedule is Monday to Friday 9 AM - 5 PM

JOB DUTIES & RESPONSIBILITIES:

- Develops, implements, and oversees case management policies, procedures, and service delivery in accordance with program standards, funding requirements, and applicable housing mandates.

- Oversees client intake, verifies eligibility documentation, and ensures compliance with program guidelines, Service-Linked Housing requirements, and other applicable mandates.

- Provides direct case management services, including housing retention, mentoring, and substance use support, and maintains accurate, organized, and compliant client files.

- Coordinates regular case management meetings and monitors individualized client service plans to support progress toward established goals.

- Coordinates and facilitates meetings, educational sessions, and training for client groups.

- Maintains current information about community resources; advocates for clients; and coordinates timely access and referrals to health care, behavioral health, substance use, education, employment, furnishings, supplies, food, clothing, and other needed services.

- Serves as the intake navigator for the Continuum of Care By-Name List and Coordinated Entry system.

- Inspects client rooms at move-in, move-out, and during program participation to ensure compliance with program, HUD, Housing Authority, and funding requirements.

- Conducts routine building rounds, monitors interior and exterior conditions, verifies that access points are secure, and addresses identified safety, security, cleanliness, maintenance, and repair needs.

- Coordinates timely maintenance and repairs; monitors maintenance and appliance service contracts; and ensures adequate supplies and the timely removal of debris and snow.

- Assigns and monitors resident chores that support facility upkeep and cleanliness.

- Serves as the point of contact for after-hours emergency calls involving building or resident matters.

- Reviews and processes facility-related bills and invoices and assists the Director of Community Housing with developing, monitoring, and maintaining the annual program budget in accordance with established procedures.

- Ensures accurate and consistent data entry in funder-mandated and Catholic Charities databases and uses program data to monitor outcomes, compliance, and progress toward established goals.

- Prepares supporting documentation and submits accurate, timely PMM and other reports required by external funding sources and the Community Services division.

- Performs other duties as assigned.

EDUCATION & EXPERIENCE REQUIREMENTS:

- Bachelor’s degree in Human Services, Business, Management, Social Work, or a related field, or an equivalent combination of education and experience.

- Four (4) years of related experience. Preferred experience includes HUD housing programs, human services, finance, operations, business, social work, or a related area. homelessness and men with disabilities.

- An equivalent combination of education and experience may be considered.

- Strong interpersonal and relationship-building skills, with the professionalism and courtesy needed to work effectively with clients, staff, visitors, community partners, and public agencies.

- Clear and effective verbal and written communication skills appropriate for client interactions, group facilitation, documentation, and coordination with internal and external partners.

- Sound judgment, critical thinking, and practical problem-solving ability to assess needs, address challenges, manage conflict constructively, and make informed decisions.

- Strong planning, organization, and time-management skills, including the ability to establish priorities, coordinate multiple responsibilities, and maintain accurate records within required timeframes.

- Ability to work independently with minimal supervision while exercising initiative, reliability, accountability, and consistent follow-through.

- Ability to engage and influence others, foster collaboration, and provide appropriate coordination and direction to support effective teamwork and workflow.

- High degree of discretion, integrity, and professionalism in handling confidential information and sensitive client, staff, and program matters.

- Safety awareness and capacity to recognize hazards, follow established protocols, and support a secure environment for clients, coworkers, and visitors.

- Proficiency in using computer systems and software to perform job function, including but not limited to, basic Windows PC, web browsing (i.e., Chrome, Internet Explorer, etc.), and Microsoft Outlook. Knowledge of other Microsoft Office applications, such as Word, Excel PowerPoint, Teams, and OneDrive is preferred.

PHYSICAL REQUIREMENTS & WORK ENVIRONMENT:

- No remote or hybrid work availability based on core and essential functions of the position. Flexibility may be required to work beyond standard hours, including weekends and holidays, as needed.

- Ascending or descending stairs, ramps or other similar structures as required to perform job tasks.

- Moving about to accomplish tasks or transitioning between different work sites, which may involve walking, standing, or traveling to various locations as required by the role.

- Adjusting or moving objects up to 50lbs in all directions, which may include lifting, carrying, pushing, pulling, or repositioning items as necessary to perform job duties.

- Engaging in communication with others to exchange information, which may involve speaking, listening, writing, or using electronic communication methods to ensure clear and effective understanding. This includes both verbal and non-verbal interactions with colleagues, clients, or external parties.

- Hearing and vision abilities, with or without correction, sufficient to drive safely and effectively observe and communicate with others.

- Driving (Agency and/or personal vehicle) is required for this position. Driver must be at least 21 years of age, with a minimum of 2 years of driving experience, and a valid driver’s license issued by the state of residence. There can be no state issued restrictions on the licenses that would impede drivers’ ability to operate the vehicle as required by the Agency, and there must not be more than three (3) points on their driving record.
