RN - Clinical Team Coordinator/Educator (Inpatient, Registration, Dietary, and Case Management) (Full-Time)

The RN - Clinical Team Coordinator/ Educator (Inpatient, Registration, Dietary, and Case Management) plans, provides and/or supervises the nursing care of patients in an efficient and caring manner. Coordinates care planning with other departments within the hospital. Adheres to standard precautions and is compliant with infection control policies. Will work as a charge nurse or floor nurse when needed due to low census staffing, coverage for breaks, or unexpected absences. Works independently with minimal supervision. This position reports to the RN - Clinical Nurse Manager (Inpatient, Registration, Dietary, and Case Management).
RESPONSIBILITIES:
- Provides leadership while working collaboratively with other care givers in the department and within the hospital on new and existing procedures and equipment .
- Monitors and maintains productivity including completion of daily assignments according to acuity and staff mix to ensure quality care.
- Maintains awareness of resource utilization on assigned patients and correlates to the clinical plan of care.
- Maintains communication with the manager regarding department concerns.
- Assist manager with staff coaching, counseling, annual performance reviews, and corrective action, including completion of informal and formal corrective actions.
- Plans and coordinates staff education and in-services including monthly education days, one on one education if needed, all while collaborating with Manager and specialty Team Leaders.
- Oversees onboarding new staff members and tracking completion of orientation checklist in a timely manner.
- Ensures completion by staff of required Advanced Cardiac Life Support, and Basic Life Support.
- Collaborates with Case Management to ensure admission status and patient classification (Inpatient vs Observation) is appropriate for the patient’s plan of care.
- Collaborates with physicians and based on patient care plan and condition, recommends appropriate consultations or ancillary services that advance patient progression. Works with the entire care team to ensure the patient is receiving the appropriate level of care and progressing according to plan.
- Assists with obtaining information and completing audits to obtain statistics, reports, and records for purposes of evaluation, continuous improvement, and reporting hospital profitability.
- Assists Manager with Weekend or Night Shift On-Call Responsibilities.
- Assures proper maintenance of clinical records in compliance with local, state, and federal laws.
- Performs the functions incorporated in the registered nurse job description.
- Assists with scheduling and payroll functions for the department.
- Other duties as assigned.
QUALIFICATIONS:
- Graduated from accredited school of nursing.
- High School graduate or equivalent.
- Licensed as a Registered Nurse in the State of Ohio.
- ACLS, BLS and certifications
