Job Summary:
The MDS Coordinator is responsible for overseeing the accurate and timely completion of Minimum Data Set (MDS) assessments and associated care plans for all residents, ensuring compliance with federal and state regulations. This role coordinates interdisciplinary care planning, supports accurate reimbursement through RUGs and PDPM processes, and ensures high-quality resident care through effective documentation, collaboration, and staff education.
Duties and Responsibilities:
1. MDS Assessment & Care Plan Coordination (40%)
- Coordinate and oversee the completion of all MDS assessments (5, 14, 30, 60, 90 days, and significant changes) within required timelines.
- Ensure accuracy and compliance of assessments and related documentation in accordance with CMS guidelines.
- Lead the interdisciplinary team in developing individualized care plans that address residents’ physical, psychosocial, and functional needs.
- Maintain schedules for assessment reference dates (ARDs) and care plan meetings, ensuring timely transmission of MDS data.
2. Clinical Oversight & Quality Documentation (25%)
- Review resident charts and nursing notes to ensure documentation supports skilled services and care plan goals.
- Monitor and evaluate the quality of clinical documentation to ensure accurate coding and reimbursement.
- Provide oversight of skilled nursing and therapy documentation to meet regulatory and clinical standards.
- Collaborate with nursing and therapy teams to ensure care is delivered according to assessment findings and established plans.
3. Interdisciplinary Collaboration & Education (10%)
- Work with nursing, therapy, social work, and dietary staff to coordinate and implement comprehensive care plans.
- Conduct pre-admission screenings and coordinate with the admissions team for appropriate placement and case mix management.
- Educate nursing and CNA staff on documentation standards, Medicare criteria, and care plan interventions.
- Participate in Quality Assurance and Performance Improvement (QAPI) initiatives and interdisciplinary meetings.
4. Regulatory Compliance & Reporting (10%)
- Maintain compliance with federal, state, and facility policies related to assessments, care planning, and documentation.
- Prepare for and participate in state and federal surveys, audits, and quality reviews.
- Ensure accurate and timely submission of MDS data to meet regulatory requirements.
- Collaborate with billing and administrative staff to verify reimbursement accuracy and address discrepancies.
5. Compliance and Training (10%)
- Follow all HIPAA privacy regulations, maintaining confidentiality for residents and staff.
- Attend all mandatory in-services, meetings, and training sessions.
- Provide exceptional customer service through compassionate and empathetic care to residents, patients, and families.
- Adhere to all facility policies and professional nursing standards.
6. Other Duties as Assigned (5%)
Qualifications:
- Education: Graduate of an accredited school of nursing.
- Licensure: Current RN license in the state of practice.
- Certification: Maintain current CPR certification.
- Experience: Minimum two (2) years of nursing experience.
- Skills: Strong knowledge of the RAI process, MDS 3.0 guidelines, PDPM reimbursement, and interdisciplinary care planning. Proficiency in electronic health records (EHR) and MDS software. Ability to make sound decisions, manage time effectively, and work both independently and collaboratively.
Physical Requirements:
- Ability to sit, stand, and walk for extended periods.
- Ability to perform repetitive tasks and physical activities, such as bending, kneeling, and reaching.
- Ability to lift up to 10 pounds.
- Work is performed in a healthcare environment, with exposure to infectious diseases, bodily fluids, and potentially hazardous materials.
