Momentum is seeking a Community and Family Engagement Manager to support our adult day program. As the Community and Family Engagement Manager, you’ll help coordinate person-centered services, support staff development, collaborate with families and community partners, while creating opportunities that promote independence, inclusion, and quality of life.

Community and Family Engagement Manager Responsibilities:
- Coordinate individualized day program services, goals, and activities that promote independence, personal growth, community integration, and quality of life for individuals with intellectual and developmental disabilities.
- Oversee intake, pre-admission visits, new admissions, and required meetings while collaborating with regional centers, families, and internal teams to ensure smooth transitions and coordinated services.
- Train and guide staff on individual support needs, goals, behavior plans, Sandata, documentation, and program procedures. Provide direct support and coverage as needed.
- Develop community partnerships and maintain positive relationships with families, conservators, staff, and service providers to promote inclusion and effective service delivery.
- Maintain accurate individual records, charts, reports, and required documentation while ensuring compliance with Momentum policies, HIPAA, and applicable Title 17 and Title 22 regulations.
- Support staff development, improve program operations, identify and report potential risks, implement appropriate safety and crisis intervention procedures, and ensure continuity of services.
Community and Family Engagement Manager Qualifications:
- Bachelor’s Degree with an emphasis in the areas of education, psychology, social services, or related field, highly preferred; relevant work experience may be substituted for education.
- Four (4) years’ experience working directly with developmental and/or disabilities in individual and/or group settings preferred.
- Two (2) years of experience with administrative responsibilities and/or supervisory experience.
- Community Care Licensing (CCL) Administrator Certificate holder highly preferred.
- Must maintain valid and current proof of legal right to drive in California; required.
- Ability to work in a fast-paced environment.
- Strong written and verbal communication skills.
- Ability to communicate and work professionally and cooperatively with others
- Ability to run, push, pull, bend, squat, kneel, lift up to 50 lbs., and transfer individuals
- Successfully pass all post offer, pre-employment screening requirements, including criminal background check, physical exam, drug screening, and provide proof of eligibility to work in the United States
