Program Coordinator

Scope: On a day-to-day basis the Program Coordinator, QIDP will provide face-to-face contact with persons served; establish positive relationships with guardians and Case Managers and communicate with them regularly; complete tasks associated with the ICP in a timely manner; monitor the implementation of programs; write treatment plans, reports and assessments; participate in team meetings; assist consumers in obtaining and maintaining benefits and health services; participate in the employee interview, hire and training process; train, supervise, and provide support to CSCs; conduct programmatic and environmental audits; operate within budgetary guidelines; develop and monitor employee schedules within budgeted hours; participate in on-call responsibilities; participate in planning and implementation of activities related to marketing and service expansion.
Accountabilities: Under the guidance and direction of the Director, the Program Coordinator, QIDP will:
- Implement the ICP as authorized and agreed upon by IDT members.
- Ensure that eligibility for services provided by ASI is established and maintained.
- Oversee and quality-assure the collection and storage of client information related to eligibility, habilitation planning and program development and implementation.
- Develop service plan and transition plans and oversee the implementation and monitoring of services.
- Develop, write, monitor and revise habilitation plans based on the needs and wants of the individual in service.
- Develop and oversee the development of progress reports related to provision of services in accordance with company standards.
- Provide training to employees on implementation of the habilitation plan and provide ongoing monitoring of outcomes from the plan.
- Provide training and supports so persons served may live as independently as possible, including response to dangerous or emergency situations.
- Coach employees and persons served in the development of daily living skills training including:
- Establishing positive relationships with community members
- Identifying activities of personal interest
- Facilitating client participation in community activities
- Identifying specific training activities to participate on an on-going basis
- Mobility training or assistance aimed at enhancing movement, accessing and using public transportation, mastering the use of adaptive aids and equipment
- Document services provided to demonstrate achievement towards individual goals and outcomes.
- Participate in the planning, coordination, and presentation of employee training including New Employee Certification, and, Continuing Education.
- Other duties as may be assigned by the supervisor.
Specific Requirements:
- Bachelor’s degree in Human Services or related field
- Experience in the oversight and direction of others
- Must have valid driver license and provide proof of vehicle insurance
- Must successfully pass state-mandated background checks and pre-employment screenings
- Must demonstrate the ability to read and write English at the level needed to read programmatic materials and provide written documentation as required
- Must successfully complete ASI orientation training within thirty (30) days of hire
- Must successfully complete 2-hour Dependent Adult Abuse Mandatory Reporter training within six (6) months of hire
- Must possess interpersonal skills necessary to work productively with other people
Physical Requirements
- Successfully complete the employee physical and health screening by the ASI-approved provider
