POSITION SUMMARY
Case Managers provide an array of services that address the individualized mental health, alcohol and drug needs of the person(s) served. They are directed toward adults, children, adolescents and families and will vary with respect to hours, type and intensity of services, depending on the changing needs of each individual.
The case manager is one member of a client’s treatment team (those individuals responsible for developing the treatment plan and providing the services). The case manager forms a partnership with the client with a shared goal of improving the client’s quality of life, focusing on identifying and accessing needed resources, increasing independence in living arrangements, improving education/vocational functioning and increasing a network of natural and community social supports.
Specific to treatment court case managers:
- Ensure that treatment recommendations are received by the Court within 10 business days of the referral
- Actively participate in all scheduled treatment court staffings and hearings
- Prepare weekly written reports on the progress of treatment court clients
- Coordinate care with treatment court team, to include attendance at treatment sessions, results of drug tests, and discharge planning
KEY RESPONSIBILITIES
- Maintain and coordinate the delivery of necessary evaluations (e.g., educational, psychiatric, psycho-social, medical, vocational, drug and alcohol) and assessments in order to identify and assist in the provision of those services necessary to meet basic human needs for food, clothing, shelter, medical care, personal safety, financial income and mental health care including outpatient counseling services and inpatient care as needed.
- Coordinate and participate in the development of an individualized, comprehensive treatment plan and ensure consumer documentation is consistent with OHMHAS standards.
- Provide coordination and assistance in crisis intervention/emergency services and stabilization.
- Assist persons served in achieving their treatment plan objectives and maximizing their independence and productivity through support and training in use of personal and community resources.
- Link and advocate with other agencies to ensure that the person served receives necessary services.
- Reassess the person’s served needs at least once every 90 days or more frequently if needed or requested by the individual (or their guardian if applicable), or based on clinical needs.
- Assist the person served to increase social support networks, including relatives and friends.
- Provide services in the least restrictive settings and lowest levels of services first, with progression into more intensive services dependent upon documented lack of success at lower service levels.
- Provide or coordinate the delivery of support, assistance, consultation and education related to the mental illness and/or alcohol/drug addiction illness to the family members, significant others and friends of the person served when appropriate. Recognize and encourage the participation of natural supports for persons served.
- Maintain all documentation as maybe required by the agency in its policies & procedures or by regulatory bodies. This includes having paperwork turned in on a timely basis.
- Abide by and follow the agency Code of Ethics.
- Focus on the individuals’ strengths rather than the pathology of their illness.
- Attend and take part in regular supervision; which may include participation in group problem solving, presenting cases for review, and sharing information on resources in the community.
- Provide the majority of services in the community, based on the person’s needs. Typically, 80% of client contacts will be spent in the community with those persons being served and/or their significant others.
- Maintain constructive, non-judgmental working relationships with individuals served, agency staff and community partners.
- Participate in the SUD treatment CPST team to assist other CPST workers to ensure services are accessed and by covering for others when necessary.
- Ensures that productivity is at expected levels.
- Maintain an “active” caseload by accepting, transferring and closing individuals in a timely manner.
- Maintenance of consumer confidentiality.
- Prompt and regular attendance. Be willing to work a flexible schedule to meet the needs of the person’s served.
- Maintains agency-required annual training including, but not limited to, client rights and grievances, safety training, infection control, and others as required by OHMHAS.
- Remains free from citations by other providers or by consumers for unethical practice and meets standard for own profession.
- Adheres to all organizational and departmental policies, including compliance with all behavioral and ethical expectations.
