Job Summary and Responsibilities
Acentra Health is looking for a Clinical Assessor, Mid to join our growing team.
Job Summary:
The purpose of this position is to complete needs-based eligibility face to face assessments for patients who are applying for these Medicaid-funded personal care services provided in their home or in adult care or supervised living homes. Clinical Assessors also complete face-to-face, clinical assessments of individuals with serious mental illness (SMI), intellectual disability (ID), or related conditions (RC) admitting into nursing homes. These preadmission screening and resident review (PASRR) Level II evaluations typically occur in nursing home or hospital settings.
- This role requires a valid driver's license and travel to various locations, which may include extended periods of driving
Responsibilities:
- Ensure accurate PASRR evaluations and recommendations through accurate clinical assessment of the patient, relevant records, and collateral sources of individuals.
- Assess the appropriate level of care and service needs based on the patient’s physical, behavioral, and psychological functioning.
- Complete medical eligibility assessments (MEA) on time and within scope of practice to meet all contractual requirements.
- Strong typing and computer skills required with proficiency in Microsoft Word and Outlook.
- Ensure compliance with program workflows.
- Ability to complete person-centered assessments with meaningful and appropriate recommendations for level of care.
- Excellent clinical experience and knowledge of psychiatric disorders as defined by the most current Diagnostic and Statistical Manual in use in New Hampshire.
- Knowledge of standards of practice related to mental health, intellectual and developmental disabilities, geriatrics, and other disorders typically requiring long term care services and supports.
- Work collaboratively with healthcare professionals, individuals and families, and other community supports.
- Communicate effectively with individuals and groups through spoken, written, and electronic media.
- Ability to attend to detail, effectively prioritize, and execute tasks in a timely manner.
- The list of accountabilities is not intended to be all-inclusive and may be expanded to include other duties that management may deem necessary.
- Read, understand, and adhere to all corporate policies including policies related to HIPAA and its Privacy and Security Rules.
Qualifications
Required Qualifications/Experience:
- Active unrestricted RN licensure with ability to practice in New Hampshire
- Resident of the State of New Hampshire
- Minimum of two years' experience in a clinical healthcare setting
Preferred Qualifications/Experience:
- Associates or Bachelor's degree in related field

Experience in Lieu of Degree
For non-clinical roles, or when not required by the contract specifically, the Company acknowledges that practical, hands-on experience can provide skills and competencies equivalent to formal education. As such, in cases where a Bachelor's degree may be required, the Company will accept a minimum of six (6) years of directly relevant professional experience in lieu of a degree. In instances where the candidate has an Associate's degree, the Company will accept a minimum of three (3) years of directly relevant professional experience in lieu of the Bachelor's degree.

Pay Range
USD $32.36 - USD $38.00 /Hr.
