- Join a family-oriented, tight-knit clinic with an established team of 4 physicians and 3 APCs
- Each clinician is paired 1:1 with a Medical Assistant
- Patient population ranges from newborns to the elderly, covering all ages.
- The clinic offers lab, x-ray, behavioral health and urgent care.
- Deliver comprehensive, patient-centered primary care, including high-quality diagnostic and treatment services, to improve the health and well-being of the community.
- Coordinate seamless care transitions for patients, managing specialized consultations, inpatient admissions, and collaborating with case management for successful discharge planning.
- Lead or actively participate in continuous quality improvement initiatives to enhance the efficiency and cost-effectiveness of care delivery within the clinic and the wider system.
- Maintain complete and timely patient documentation and electronic medical records to ensure compliance, accurate records, and support the financial integrity of the practice.
- Ability to practice autonomously and demonstrate comfort managing a solo outpatient primary care panel within a clinic environment

Licensure / Certification / Registration- Required Credential(s):- One or more of the following:- Physician MD/DO credentialed from the Minnesota Board of Medical Practice obtained prior to hire date or job transfer date.
- Physician MD/DO credentialed from the Wisconsin Medical Examining Board obtained prior to hire date or job transfer date.
- Preferred Credential(s):- Advanced Life Support. American Heart Association or American Red Cross accepted.
Education- Doctor of Medicine (MD) or Doctor of Osteopathy (DO) required.
