Transition of Care Coordinator

Centrum Health · Doral, Florida, United States

WHO WE ARE

NeueHealth is a value-driven healthcare company grounded in the belief that all health consumers are entitled to high-quality, coordinated care. By uniquely aligning the interests of health consumers, providers, and payors, we help to make healthcare accessible and affordable to all populations across the ACA Marketplace, Medicare, and Medicaid.

NeueHealth delivers clinical care to health consumers through our owned clinics – Centrum Health and Premier Medical – as well as unique partnerships with affiliated providers across the country. We also enable providers to succeed in performance-based arrangements through a suite of technology and services scaled centrally and deployed locally. Through our value-driven, consumer-centric approach, we are committed to transforming healthcare and creating a better care experience for all.

The Transition of Care Coordinator (TOCC) is responsible for managing the patient’s transition from an inpatient, observation, skilled nursing, or emergency department setting back to the primary care setting. The TOCC identifies admitted and discharged patients through health information exchange (HIE) admission/discharge/transfer notifications, health plan census files, state-provided feeds, and facility notifications, and reconciles those sources into a single daily transition-of-care worklist.

Working under the guidance of Clinical Leadership, the TOCC completes post-discharge outreach, schedules and confirms timely follow-up with the Primary Care Provider, ensures the discharge summary and supporting facility records reach the provider before the follow-up visit, and coordinates the home-based and ancillary services the patient needs to remain stable after discharge, including home labs, APRN home visits, home health, durable medical equipment, transportation, and disease management referrals.

The TOCC serves as the liaison between the patient, family or caregiver, facility discharge planners, health plans, and the internal care team, and is accountable for documentation that supports transition-of-care quality measures, readmission reduction, and continuity of care. Other duties may be assigned to suit the demands of the patient population, as this position is committed to the constant pursuit of excellence in improving the health status of the community.
