Full-time, benefit eligible position. Monday - Friday 7:30am-4:30pm. 1 hour lunch. 40 hours weekly.
The Obstetrics Coordinator RN is a key member of a person-centered, integrated care team, helping obstetric patients experience connected, timely, and supportive care throughout the prenatal, perinatal, and postpartum continuum. This role combines nursing assessment and education with patient navigation, care coordination, outreach, and collaboration across OB providers, hospitals, specialists, and community partners. The RN helps remove barriers to care and ensures patients have the information, resources, and connections they need to support their health and well-being.

- Coordinate patient care across the prenatal, perinatal, and postpartum continuum, including transitions between clinic, hospitals, specialists, and community services.
- Track and follow up on appointments, testing, referrals, consultations, delivery planning, and postpartum care to promote continuity.
- Provide patient outreach, navigation, education, and follow-up related to care plans, medications, testing, warning signs, delivery preparation, and postpartum needs.
- Identify barriers to care and connect patients with appropriate organizational and community resources, including coverage, transportation, language, equipment, and public health resources.
- Perform nursing assessment and telephone triage, using clinical judgment to prioritize concerns and coordinate appropriate provider follow-up.
- Collaborate with patients, OB providers, hospitals, specialists, and interdisciplinary teams to support effective communication and safe transitions of care.
- Participate in care planning for patients with complex medical, behavioral health, or social needs and coordinate follow-through.
- Maintain timely, accurate documentation and support quality, safety, education, infection prevention, and compliance initiatives.
- Provide occasional direct clinic nursing support based on operational needs.
• Associate/Bachelor’s degree in nursing required
• Minimum of one (1) year of experience as a registered nurse required
• Five (5) or more years of healthcare experience preferred
• Obstetric nursing experience preferred
• Experience with healthcare technology and electronic health record systems preferred
• Care coordination, case management, patient navigation, or population health experience preferred
• Experience coordinating care for patients who require interpreter services or face barriers to care preferred
LICENSURE/CERTIFICATION:
• Must possess and maintain a current, active Missouri Registered Nurse (RN) license
• Must obtain and maintain current CPR certification within 60 days of hire
• Must obtain and maintain certification in an approved crisis intervention methodology (CPI, MANDT, or CALM) within 60 days of hire
