At Lakewood’s 87-bed, Medicare-certified skilled long-term care facility, you’ll build relationships with residents and families while putting your nursing knowledge into practice. Our Care Center provides care across long-term care, a specialized dementia unit, and a transitional care unit, offering team members the opportunity to work with residents at various stages of their healthcare journey. Your work will help residents maintain their abilities, receive personalized care, and feel respected in the place they call home.

Your Role in Resident Care
As a Long Term Care RN, you’ll provide hands-on nursing care and clinical leadership that help each shift run safely and smoothly. Your responsibilities include:
- Assess resident needs, recognize changes in condition, and respond to situations requiring RN intervention.
- Guide care decisions and update individualized care plans to reflect residents’ needs and preferences.
- Administer medications and complete treatments with careful attention to safety and accuracy.
- Support restorative nursing programs that help residents maintain function and independence.
- Communicate changes in condition with families and collaborate with providers and interdisciplinary partners, including during hospice and palliative care discussions.
- Guide LPNs, medication aides, and resident assistants through clear expectations, education, feedback, and accountability.
- Complete timely documentation and thorough shift handoffs so the next team has the information needed to continue care.
- Participate in quality, physician, and pharmacy rounds and complete follow-up actions.
- Follow infection prevention practices, safety standards, and care requirements.

We are hiring full and part-time positions! We offer flexible scheduling to promote work/life balance. Available shifts include:
- Offering flexible scheduling
- Shifts are: 6:00AM-6:30PM; 2:00PM-10:30PM; 6:00AM-2:30PM; 5:45PM-6:15AM
- Weekends - every third
- Holidays - rotating
Additional compensation is available through:
- Competitive shift differential (PM/NOC), shift pick-up, overtime
