Volunteers of America North Louisiana began in Shreveport in 1935 with the opening of a shelter for women and children. JOB DUTIES/ESSENTIAL FUNCTIONS:

- Conduct needs assessment with client upon intake and update as needs change.

- Develop, in collaboration with the client, a comprehensive service plan with achievable goals. Service plan will be reviewed in accordance with agency guidelines and state licensing/certification requirements, and modified accordingly. Service plan will be shared with Residential Trainers to achieve comprehensive services for the client.

- Provide advocacy in obtaining services both in-house and in the community.

- Assist with obtaining resources such as SSI, SNAP, etc.

- In collaboration with Residential Trainers, provide training to client following goals developed in their service plan. Training may include money management, personal hygiene skills, grocery shopping, housekeeping, preparation of meals, laundry skills, and community integration.

- Document services and training provided in Service Point HMIS/LSNDC and client’s file daily. Documentation of services and training should follow agency guidelines and HUD requirements.

- Communicate information necessary to the well-being of clients using shift logs. Necessary information may include medication changes, missed medication, behavioral observations, changes in service plan, and other information relevant to clients’ well-being. Shift logs will be reviewed daily.

- In conjunction with the Program Director and/or Residential Trainer, plan and conduct structured activities for participants.

- In conjunction with the Residential Trainer, ensure that clients attend medical appointments as scheduled.

- In the event of illness or injury, take appropriate steps to ensure medical attention is received, if necessary. Report all illnesses or injuries to the Program Director or supervisor on call.

- Coordinate medication refills in a timely manner. If the Residential Trainer cannot obtain the refill, the Case Manager should be included for assistance.

- Transport clients to appointments or social events as needed.

- Participate in safety drills.

- Complete critical incident reports as needed. Follow policy for reporting.

- Liaise with client’s family or next of kin, notifying them of changes or concerns with the client. Document issues brought by client and/or family.

- Maintain client confidentiality per HIPAA standards.

- Identify and manage crises. Contact supervisor for needed support and guidance.

- Identify and report maintenance issues to the Property Manager.

- Develop and maintain positive relationships with agency staff members, outside agencies, and local community.

- Attend staff trainings and meetings as required by VOANLA.

- Work on special projects as assigned.

REQUIRED EDUCATION AND EXPERIENCE:

- High school diploma or GED. Bachelor’s degree in social work or related field preferred.

- Two (2) years of experience in human services field preferred.

- Experience working with homeless or vulnerable populations preferred.

- Current CPR/First aid certification required or obtain upon hire.

- Current valid driver’s license with acceptable driving record.

- Automobile liability insurance that meets minimum requirements.
