About the Role

The Care Coordination Manager is responsible for overseeing the coordination of client care, caregiver scheduling, service coverage, and day-to-day operational needs while providing leadership, guidance, and support to the Care Coordination team and Hospital at Home (H@H) Coordinators and Program.

This position plays a critical role in ensuring clients receive consistent, reliable, and high-quality care while maintaining strong communication with clients, families, caregivers, hospital partners, and internal departments. The Care Coordination Manager serves as a key operational resource for coordinators, helping them navigate daily responsibilities, resolve challenges, prioritize urgent needs, and ensure operational issues are addressed promptly and effectively.

The Care Coordination Manager will oversee daily and future schedules, monitor open shifts, respond to caregiver call-offs, coordinate replacement coverage, address service concerns, and ensure changes in client needs are communicated and handled appropriately. The Manager will also support the Hospital at Home Coordinators and Program by helping coordinate daily operational needs, communication, scheduling, service delivery, and issue resolution to promote a seamless experience for patients, hospital partners, caregivers, and internal teams.

This role works closely with and supports coordination between Intake, Clinical, Hospital at Home, Recruitment, HR, Payroll, Billing, Compliance, and Management. The ideal candidate must have prior home health or home care experience, strong leadership and organizational skills, excellent judgment, and the ability to remain calm, accountable, collaborative, and solutions-focused in a fast-paced environment.

Key Responsibilities

- Lead, support, and provide daily guidance to the Care Coordination and Hospital at Home (H@H) teams.

- Oversee client, patient, and caregiver schedules, service coverage, open shifts, and staffing needs.

- Respond to call-offs, cancellations, late arrivals, and schedule changes; coordinate appropriate replacement coverage.

- Support H@H Coordinators with daily program operations, scheduling, service coordination, workflow, and communication needs.

- Ensure clients and H@H patients receive timely, reliable, and well-coordinated services.

- Serve as a resource and escalation point for coordinators regarding client/patient needs, caregiver concerns, scheduling issues, and operational challenges.

- Communicate effectively with clients, patients, families, caregivers, hospital partners, and internal teams to resolve service concerns and maintain continuity of care.

- Support new client and H@H starts, hospital discharges, service changes, and care plan adjustments as applicable.

- Maintain accurate scheduling, EVV, client/patient, caregiver, and program records; assist with payroll and billing accuracy.

- Collaborate with Intake, Clinical, H@H, Recruitment, HR, Payroll, Billing, Compliance, and Management.

- Ensure company policies, compliance requirements, confidentiality, and documentation standards are consistently followed.

- Monitor team workflow, promote accountability and teamwork, and ensure daily responsibilities are completed accurately and timely.

- Identify operational challenges and recommend improvements to care coordination, H@H processes, communication, and workflow.

- Perform other duties as assigned to support the overall success of the AccordCare team.

- Prior professional experience in home health or home care required.

- Experience with caregiver scheduling, client services, care coordination, or home care operations.

- Strong leadership and team-support skills.

Preferred Qualifications

- Care coordination, scheduling, staffing, operations, or supervisory experience.

- Experience supporting a Hospital at Home or similar healthcare program.

- Experience working with hospital partners or clinical teams.

- Experience with EVV and home care scheduling platforms.

- Previous supervisory or leadership experience.

Compensation & Schedule

- Full-time, salaried position

- Starting salary: $55,000
